Our program uses proactive strategies to combat social isolation among individuals, including personalized, risk-based intervention and continuous outreach to build trust, engagement and adherence.
We’ve adapted our original program to help Medicare Advantage plans meet the new Star Rating measurement focused on depression screening and follow-up. We use similar data and assessments to identify at-risk individuals and create tailored intervention plans that proactively engage them, connect them to essential resources, and provide ongoing support.
Our U.S.-based compassionate Social Care team is comprised of licensed and degreed social workers. These are real humans (not AI assistants) who take time to speak with and truly understand individuals and their unique needs.
